Medical Records

Conway Regional's Health Information Management department is able to assist you in obtaining your medical records. We have several options to meet your needs.

1. Patient Portal

The portal offers patients the ability to securely view and print/download portions of their medical records, such as provider notes and diagnostic testing results. The use of the patient portal is FREE. Click here.

Patients can also invite others who are involved in their healthcare to access the patient portal. The access can be defined as billing, clinical, or both.  Click on “Account Options” to extend an invitation.

 

2. Automated System

  • For legal guardians or legal representatives requesting records on behalf of a minor/dependent or another patient

If you select unencrypted email as your delivery method, the total file size must be less than 8MB and only text documents are eligible for email. Image files (pictures, X-ray images, etc.) are too large and must be provided on a CD. 

Requestors should also be aware of the following risk of unencrypted email:

  • The information may be obtained by someone else.
  • The information can be opened and read by someone else.
  • Unencrypted information does not provide any assurance of privacy or security
  • Detailed demographic and insurance information (facesheet) cannot be sent by unsecure email

 

How long will it take to receive my medical records?

Under federal regulations, records must be provided within 30 days. We are able to complete a request for medical records within 7 to 10 business days after our office receives a request. If your request is delivered in-person, we can often process the request while you are in our office.

 

What information do I need to provide?

Records can be released to anyone that the patient authorizes, in writing. A valid authorization must contain the following information or the request will be returned:

  • Patient's full name and date of birth
  • Hospital patient ID, if available
  • Specific information being requested (e.g., type of report/information and dates of service, etc.)
  • To whom the information is to be sent (name and address)
  • The patient's signature or a patient's legal representative's signature—authorizations signed by a representative must contain a copy of the guardianship papers or power of attorney
  • Date of the signature
