The Conway Regional Well@Home program benefits patients who need ongoing monitoring of vital signs such as:
- blood pressure,
- blood glucose levels
Patients that benefit most from this service include:
- patients with cardiovascular disease
- newly-diagnosed diabetics
- patients with renal conditions and other disorders
- medical patients who are non-compliant with medication
- hard-to-manage, homebound patients.
The Well@Home equipment is programmed to remind patients to take their medicine and provides instructions on how to check their blood sugar level, blood pressure, pulse/ox and temperature. A specially designed scale can be connected to the monitor for patient weigh-ins. These readings are sent via telephone lines to the HomeCare office where they are monitored.
Patients also have access to an on-call nurse who can provide triage over the telephone if needed. If necessary the nurse will visit the patient or call an ambulance to take the patient to the hospital.
